$249 per Month
Ideal for busy families, older adults, or anyone who wants healthcare without the hassle. Our Essentials program brings personalized primary care to your doorstep — with in-home lab coordination, helpful home environment insights, and extra support managing the details. We handle the logistics, so you can focus on your health, your family, and your life.
Primary Care Plus
$575 per Month
Our plus program goes beyond the basics to bring you comprehensive care designed to give you the confidence to manage your health. Ideal for those who want more than just the basics, those with multiple complex medical conditions, and those recently discharged from from the hospital. With a little extra help, you can feel confident in managing your health.
Aging at Home

We all want to age on our terms. Our Aging at Home program is designed to grow with you as your needs change, providing extra support when necessary and helping you manage your health and your care in the comfort of your home.

In-Home Provider Visits
With health2me, you get convenient, high-quality primary care through in-home doctor visits from board-certified physicians. Whether you’re managing chronic conditions, recovering from illness, or staying on top of preventive screenings, our team provides full-scope mobile primary care services directly in your home — no clinic trips or waiting rooms needed.
Telehealth
Whether it’s a virtual check-in, follow-up conversation, or a quick question for your provider, we offer HIPAA-compliant telehealth and secure messaging for clinically necessary care. These services are billed to your insurance under standard communication-based care codes.
Vaccinations & Minor Procedures at Home
From flu shots and pneumonia vaccines to joint injections and shave biopsies, our team delivers vaccinations and minor procedures safely in your home—no extra trips needed. This convenient option helps reduce stress and hassle by avoiding travel and long waits at clinics.
Comprehensive Medical Care Covered by Insurance
All clinical services—including in-home primary care visits, care coordination, telehealth, and vaccinations—are billed to your insurance carrier according to standard billing practices. Your annual fee supports comprehensive care support, convenience, and additional non-billable benefits but does not cover insurance-billable clinical services.
